Provides leadership and supervision to case management staff. Develops and implements the systems to ensure that case management is effective in meeting the needs of clients and meets the State and Federal requirements outlined in the OARs. Member of the departments Management Team, tasked with building quality services through CM and local providers.

Essential Duties and Responsibilities

The following may not be all inclusive and other duties may be assigned.

01 Directly supervises DD case managers; 3 CM in childrens' team and 3 CM in adult team.

02 Assures quality of case management by monitoring charts, case notes and observing staff with clients and community partners.

03 Develops and implements systems to facilitate the efficiency/effectiveness of case management program and services.

04 Audits charts and services for Medicaid/waiver accountability and uses data for building systems and evaluating staff.

05 Assists in solving issues with community partners around client issues.

06 Supports CMs in solving support issues with individual's team as needed.

07 Correlates individual monitoring data to facilitate corrections at provider level.

08 Backs up Administrative duties around Program/clinical issues when Director unavailable.

09 Provides case management services to a small caseload to stay current with changes, not to impede supervisor duties and to remain accessible to CM team.    

10 Structures individual supervision and support of staff.

11 Management Team member, demonstrating leadership around Case Management Program.

12 Participate in provider development to keep resource base meeting community needs.

Education and/or Experience

Master's degree or equivalent; or four to ten years related experience and/or training; or equivalent combination of education and experience.

Work Schedule

The normal work schedule is 8.00 hours per day 5 days per week; exact schedules are assigned by management. It is the attendance standard of the employer that all employees be present and on time each shift they are scheduled to work. Full or partial absence during any scheduled shift, arriving late, or leaving early, for whatever reason, may result in disciplinary action up to and including termination of employment. Time off may be approved if allowed by applicable policies covering illness, injury, or other personal emergency.

Supervisory Responsibilities

Directly supervises employees in the Case Management. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.
