Summary

The position provides targeted case management services to Developmentally Disabled (DD) consumers and their families by identifying consumer needs and developing a plan to access available services to support people with developmental disabilities in the community.

Essential Duties and Responsibilities

The following may not be all inclusive and other duties may be assigned.

01 Provides advocacy and protective services to developmentally disabled persons.

02 Assists consumer/team in developing and implementing a support system for the specific consumer's needs as a result of a Level of Care, Adult Needs assessment and corresponding Individual Support Plan (ISP).

03 Authorize Medicaid expenditure and monitor service hours through Plan of Care (POC).

04 Monitor ISP for effectiveness and Medicaid standards as outlined in the OAR's.

05 Documentation to demonstrate waivered and non-waivered services are being completed in compliance with plan and meets billable level to support the position.

06 Check Medicaid authorized expenditure against plan of care for K plan services.

07 Facilitate supports in response to consumer acute need.

08 Support/educate providers in implementing supports per plan of care, ISP and OAR's for that service element.

09 Develop and maintain effective working relationships with community partners and stakeholders.

10 Represent ID/D on community boards/councils as directed by Program Supervisor.

11 Provide case management to eligible consumers not requesting waivered services or not eligible for waivered service or State Plan services.

Education and/or Experience

Bachelor's degree from four-year college or university in related field; Bachelor's degree in any field with one year human services experience; Associates degree in related field and 2 years related experience; 3 years of related experience.

Experience with people with Developmental Disabilities.

Work Schedule

The normal work schedule is 8 hours per day 5 days per week (will require some weekend and evening hours); exact schedules are assigned by management. It is the attendance standard of the employer that all employees be present and on time each shift they are scheduled to work. Full or partial absence during any scheduled shift, arriving late, or leaving early, for whatever reason, may result in disciplinary action up to and including termination of employment. Time off may be approved if allowed by applicable policies covering illness, injury, or other personal emergency.
